New Patient Information

We hope that you find a new home with Southdale Internal Medicine. If you choose to see us, there are a few things that we request you keep in mind to assist us in providing you outstanding service.


• Arrive 15 minutes early for your first appointment so we may get you checked in.
• If you are unable to make your appointment, we request that you cancel with as much notice as possible.
• Bring all current insurance cards.
• Bring co-pay with you, as we are required to collect this at the time of your appointment.
• Bring or send any pertinent medical records from other providers.
