Insurance & Billing

• We accept most insurance plans.
• Your insurance card must be presented at every appointment.
• You are responsible for co-payments at the time of your appointment.
• For your convenience we will bill your insurance company or health plan. We will bill you for the balance due on your accounts once all payments and adjustments have been made.
• Certain health plans require you to see only their preferred providers or obtain care only at their facilities. Please contact your health plan to verify that our clinic would be covered under your health plan.
• If you do not have medical coverage of any kind, you will be personally responsible for the care provided. Arrangements for payments may be made with our billing staff.

For patients with a managed care options we are in the following networks:
